House Manager

This home requires an experienced, dynamic and effective manger to lead and supervise a staff team in supporting two Individuals with complex needs.  Working collaboratively with a professional network is essential to this position.   Only serious applicants that meet the stated qualification need apply.

Job Description

  • Actively supports the development of the SHSS Service Plan in collaboration with the child or youth and their SHSS Care Circle.
  • Is able to confidently accesses and navigates thru the SHSS Service Portal to ensure compliance in all practice areas
  • Provides supervision that supports consistent quality Service, including supporting Service Provider Personnel and Subcontractors with implementation of Strategies established in children and youth’s SHSS Service Plan
  • Actively participates the development of the child’s and youth’s transition plan (per the SHSS Service Plan), drawing on other experts where necessary and in collaboration with Guardianship Workers (if applicable).
  • Coordinates the child’s and youth’s care team to ensure integrated and holistic assessment and planning.
  • Provides leadership, guidance and participates with Service Provider Personnel and Subcontractors, families and others in planning and providing individual plans, case conferencing, case management and the preparation of related documents and reports.
  •  Facilitates training for Service Provider Personnel and Subcontractors. 
  • Facilitates access to supplemental services to support children, youth, their families, and Service Provider Personnel and Subcontractors.
  •  Liaises with the community, government, families, officials, and professionals and promotes community involvement in the program.
  • Collaborates with the individual, their families and other stakeholders to pro-actively create and promote a plan of care which affords opportunities for inclusion and full participation in both the community and the individual’s home life.
  • Evening and weekend work may be required.

Requirements:

QUALIFICATIONS                                                         

Education

  • Undergraduate Degree in Psych, Social Work, Counselling or equivalent, plus
  • Diploma in related human/social service field will be considered (depending on combination with background experience))
  • First Aid training (Condition of Hire)
  • CPI and/or Mandt certification and training and/or :
  • Specific training in behavioural modification, children under 12 years of age, attachment based experience, and relationship-based interventions (Trauma informed practice)

Background and Experience

  • Minimum 3 years related experience.
  • Minimum 1 year administrative or supervisory experience is required
  • Demonstrated working knowledge of community-based resources
  • Familiarity with MCFD Plan of Care (CPOC) and/or CARF Person Centered Planning (PCP)
  • Understanding and insight into support needs for individuals with diverse abilities , behaviour challenges and mental health issues
  • Familiarity with following diagnosis/conditions: ASD,ADHD,ADD, anxiety, OCD, PTSD,SIBs, mental health issues
  • Insight into Children’s services including social-recreational and developmental needs
  • Experience in collaboratively working with stakeholders in the development of person centered planning documents
  • Group Process and Facilitation Skills/ Conflict resolution
  • Experience in accessing community resources (employment, recreation)
